Working Papers

If a student damages their pouch to a point where it does not lock or cannot secure the phone properly, they will have pay a $15 dollar fee to have it replaced.
If a student does not pay for the replacement, their phone will be collected each day.
There will be random pouch checks by the deans and staff to ensure the condition of the pouch is acceptable.
Parents will be informed by the dean’s office or a teacher if a student has a damaged pouch.
Please encourage your child to keep the pouch in good condition and to report any issues with the locking mechanism.

Working Papers

In order to get working papers, please do the following:

Complete the top (Part I only) of this form Employment Application Form.pdf
Physical Fitness Form -Working Papers.pdf signed and stamped by the doctor's office
A copy of your Birth Certificate, Passport or any Government Photo ID document

Once ALL of the forms above have been completed, then you can bring it to Ms. Ortiz into Main Office, Room 101.

Please note that it takes 24 hours to process.
